a listening ear and emotional support in times of need. Sharing the burden with
our concerned staff will hasten rehabilitation and encourage healing.
Discharge Planning - For specialized services which may be required after leaving
the hospital, the staff can help make arrangements for assistance such as:

Medical Equipment to use at home
Nursing Home placement
Rehabilitation Care Facility
Psychiatric Care
Nutritional Services
Financial Services Support - Our staff is OSHIP certified wtih Medicare, and
knowledgeable about different types of assistance which, may be available to patients,
including Medicare, Medicaid, Disability and other Community and Government based
assistance programs.
Advanced Directives - Many people prefer to make specific healthcare decisions before
serious illness strikes. Advanced Directives are documents which provide guidance
and the authority for the person(s) you select to make medical decisions for you.
If you want to learn more about, or complete an Advance Medical Directive such as
a Living Will or Power of Attorney for Health Care, we can provide the forms and
guidance necessary.
Patient Advocate - Your thoughts, concerns and feelings are extremely important
to us. We recommend that patients first attempt to resolve any problems with the
department in which the problem occurred. If these attempts are unsatisfactory,
call the Patient Advocate. This will give us an opportunity to solve the problem.
Your concerns are taken seriously. Patient feedback is reviewed by hospital Leadership
and is used to enhance policies and improve services.
