If you have problems accessing your records through MyChart, please call 937.840.6507.
If you see physicians or providers in our Outpatient Specialty Services clinic, records for those visits might only be available directly from the physician’s office or through another patient portal. Please contact medical records prior to coming to pick up your medical records so we can direct you accordingly.
